Home Test Kits for HbA1c and Colorectal Cancer Screening
Specifically eligible members in Alaska will be sent home test kits before the end of 2023. This annual program aims to meet our members where they are and make it easier for them to receive these important screenings.
Please note not all groups and populations are eligible. Consult with the member engagement team if you have any questions on eligibility or participation for your specific group or population, before communicating to your group.
Based on eligibility, members will receive either a FIT (fecal immunochemical test) kit and/or a diabetic HbA1c test kit. There is no cost to members who receive a kit through this program.
- Individuals aged 45-75 years of age should be screened for colorectal cancer at regular intervals, for early detection of abnormalities.
- Individuals aged 18-75 with a diagnosis of type 2 diabetes, should receive a Hemoglobin A1c test once every 12 months.
Members will receive up to 2 reminder phone calls and 1 reminder letter to complete their test kit. Premera eligible commercial membership Alaska state will also receive a text message reminder.
